Spay/Neuter Appointment Form
This form is only for scheduling an appointment for your pet to be spayed or neutered with EMANCIPE+. This form is not for scheduling a wellness appointment (for “just” shots, etc). For wellness information please go here. If you would like to book an appointment for low cost sterilization (spay/neuter) for your pet, please complete the Appointment Request Form below. An EMANCIPE+ representative will contact you via e-mail to help you with scheduling this appointment. Please remember your appointment isn’t confirmed until you get a confirmation e-mail. No appointment is necessary for the Mobile Clinic Free Days held each week Thursday – Saturday.
